[Carbon-dev] Sample Chad in appserver

I just tested Chad sample in a fresh WSO2 AppServer-4.0.0 distribution and I
was able to login to Chad UI without any issue. See the attached screen
shot.
There is a limitation in this sample if you change HTTP port, you cannot
login to ChadUI. You can try this out in a fresh AppServer build without
changing default ports.
